Is it time for accutane?

Been battling adult acne for about 4 years now and it’s just steadily getting worse. This has been my baseline. I’ve tried all the topicals. Accutane definitely scares me but I feel like I’ve seen people do it with skin that’s actually better than mine… how was your skin when you decided it was time for accutane?
